Why are you modifying the filter policy, and what do /Shibboleth.sso/Session and the transaction log say it's caching in the session?

Separately, I won't debug proxy configurations. If you can run a CGI script locally, you can check for the headers or environment variables locally being set. After that works, it's an Apache issue.
