Should the installer automatically create the Shibboleth site under the Default Website, and copy the relevant files into C:\inetpub\wwwroot\   ?

It is probably barely 30-40% likely that it can actually configure IIS successfully, and that's probably closer to 20% on a site with any complexity. If it doesn't work, then you have to manually configure all of the necessary integration touchpoints, as described on that page. So, that's where you are.

When handlers don't work, the script mapping isn't there. If the GUI says it is, it's lying, which it in fact does fairly often.
