So I think I need to do a separate intercept flow that:
        - checks for the attribute
        - if missing, shows a form-view for the user to set security Q&A
        - on submit, write the data to the directory
        - send user back to the authn flow

We have developed a post-intercept flow to do this (though storing data is handled by an external application). You'll have to squint to see beyond the VT-specific implementation, but I believe it would be helpful to review. I'll try to make available today.
