Your IDP reported an error to the SAML SP, so that's where you'd look
(i.e., your idp-process.log around the given time).

If this is a new IDP it's probably misconfigured, e.g. by being
configured with the RemoteUser login handler (in v2 terms) and nothing
protecting the appropriate endpoint.
