SP-initiated SSO implies that the SP is making the request so obviously it's in control of that decision. Beyond that, it's a software, application, and deployment specific question. I can only speak to Shibboleth, and any Shibboleth SP with both options that's not already using SAML 2 is a self-evident example of what I'm talking about because it's being artifically contorted to do other than what it would naturally do.

You're supposing from that that Shibboleth deployed in a natural/default state is the only case, and it's just not.
