Troubleshooting Logout in SP

I am having an issue with one of our environments on not clearing the shib sp session on logout (local).  We tried 2.5.3 and 2.5.4 on Windows (behind Apache 2.4) with the same result.  It doesn't send the set-cookie to clear the cookie in the browser either.  I have logging at debug for nearly everything for native and shibd and not seeing anything that is being called for Logout.  There are messages that the Logout handler is initialized.
