The form asks for issuer as well and accepted urn:mace:incommon:alaska.edu
but the Salesforce documentation* states that the required EntityID field
is "the entity ID for the identity provider."

I will try that. I considered but dismissed the idea that I might be able
to craft a second EntityID for my IdP as being ridiculous - however did
that option get implemented?  Going to make my migration to V3 more
challenging if I have to use this V2ism.
