IdPv3 beta 1 & testshib

Hi David, 
There is a bug in the MD generator. 
It is fixed but not in a release yet.

There is a thread about a week ago that covers this if you can find it, has testshib in subject. :)

Remove the SLOut endpoint definitions and move the NameID elements up above the previous elements and you should be good to go.
