That screams out that you've virtualized your site, and haven't done it
correctly. Why are you setting scheme and port? How are you deploying this
site? You don't set those attributes unless you're virtualizing the site
with a proxy or load balancer or something like that.

I've told you what you need to do. Continuing to post and not tracing the
traffic yourself will not solve your problem.

The shib_sp_session cookie is not set or not returned after it's set, or
in rare cases the cookie is sent but the session is invalid when it's
used, which is noted in one of the logs.
