REMOTE_USER

We are testing a Shib IdP with multiple SPs running authentication against
an external MySQL DB successfully.
We have a requirement to add a further SP where the application needs to
know the ID of the logged in user using REMOTE_USER (Apache httpd on Linux).
I have repeatedly reread the documentation on this subject but have to
admit I am still confused :(
Would somebody please be kind enough to walk me through the steps required
to pass the authenticated username (as input by the end user) through from
the IdP and the Sp to Apache.
Currently I can see the following but not REMOTE_USER
Many Thanks
