App pool crash / filter unloaded - ShibSP 2.5.1 on Windows 2008 Server SP2

By which I take it that the filter is operating, logs are appearing, and
so forth.

I assumed at the time it wasn't running at all. If it's running, it
doesn't crash for others, so that means it's an environmental problem, but
I wouldn't assume it's permissions. If it weren't running, and still
crashing, I would tell you the ACLs really aren't correct for the app
pool, but that doesn't fit here.

I literally have no idea how to get a stack trace on 2008, but if it's
possible, the debug files can be substituted as I described. That doesn't
really mean much. Without a debugger, a shutdown problem would probably
not manifest in a useful way in a dump.

If there are any other extensions or unusual options set, then I would
start with those and if they can be narrowed down to something
reproducible then I can fix it.
