The error occurs if the site instance ID that IIS reports to the module is
not in the set that is mapped in shibboleth2.xml. So generally it means
the <Site> mappings are wrong or incomplete. It often occurs when IIS
isn't fully restarted after changing them. Most people don't understand
how to restart it.
