The cookie that tracks the state of the login process is missing or
something has destroyed the context in between steps. I don't know how you
have managed to make that happen.

Then your IE browser is misbehaving in some way or isn't accepting cookies.

None of that addresses the failure in the LDAP module. An NPE is a bug of
some kind, but most likely the login.config is malformed in some subtle
unusual way.
