FWIW, the issue we've seen is installing the SP on Windows machines with a hyphen in the hostname.  Openssl chokes on generating the cert, so you end up with an sp-key.pem, but no corresponding sp-cert.pem.  It's been there for a few versions at least, but I haven't checked the latest version to see if it is still there.
