Oddly it sounds like I was right at the time, but reading it now I don't know what made me jump to that exact conclusion. It certainly could be other bugs, I suppose. But I would imagine since the message detail is exactly the same, it's probably the same issue.

I guess that's up to the IdP, but that doesn't seem likely.
