Modifying uApprove Shibboleth login process

Hello all,

i want to change the process of logging in.
Let me explain.

I am using shibboleth and uApprove.
At the moment the user gets shown the attribute release content every
time he logs in.

Now i want an additionally small checkbox on the bottom of the
login-screen.
This checkbox should have a label like this: 'Show my attributes'.

When he logs in and did not check the box, then he should not get the
'attribute release content'.
But when he checks the box before logging in, then he should get the
'attribute release content'.

Could you help me and tell me which files i have to edit?

I did not want to use the solution of uApprove shown below point 4.1

My mother-language is german, so maybe there are some german people who
could help me?!
