Well, aside from your REMOTE_USER issue, he identified your endpoint
problem; your metadata is wrong, or the request is being made with an
inappropriate endpoint index. One or the other, nobody else can make that
determination.

As far as REMOTE_USER is concerned, that has nothing to do with Shibboleth
or really this list at all. It's up to you and your container. If you're
using Tomcat, I suggest you try their list if you can't get their
container managed authentication to work.
