If you're manually managing the federated relationship, then that's on you
(and them). That's why it's a good idea to leverage federations that
manage metadata for you.

No, SPs will generally not notify you. They don't often understand any of
this. Not using a federation shifts costs onto each party that cares
enough about the problem, but more often it simply gets ignored, people do
the minimum, and stuff breaks when it breaks. One's tolerance for that
will vary. I have more or less none (I'll bet you're shocked).

On campus is a different story, but I assume you're talking about third
parties.

What you need to understand is what you're assuming as a responsibility so
you can either meet it, or explicitly choose to ignore it.
