SP issue: Shib-Identity-Provider not set for some IdPs

Until or in 2.1 there was a bug where Shib-Identity-Provider
sometimes was not set. I can't recall the specifics but either way
they shouldn't be running 2.1 anyway.
Some SP we federate with had this problem back in Nov 2011 when using
stock Debian/Ubuntu packages instead of the backports. Upgrading
solved the issue (and will prevent any security issues with those
old releases). So the biggest help would be pushing them to
upgrade. Version 2.5 is around the corner...
