Inbound message issuer was not authenticated

The most common cause of that is that your Servlet container is
fronted by a web server that is not passing the certificates used by
the SP to the IdP.  If you do have your Servlet container fronted by a
web server check the documentation for whatever mechanisms your using
to hand off the incoming request to the container.
