Gary,

Your entries are associated with your OpenIdP account(maxwell at openidp.org) but you seem to be logging in using your ProtectNetwork account(maxwell_g at idp.protectnetwork.org).  If you can't login with OpenIdP or that is not your OpenIdP account, I can reassign your provider entries to your ProtectNetwork EPPN.
