Apache 2.4.3 with Shibboleth 2.5.1 producing 500 error instead of providing DirectoryIndex page

It appears that I can not file a new issue, since when I try to login via my 
school (University of Oregon), I get the message that says:

    *Login Unsuccessful*

    In the interests of protecting your privacy, your Identity Provider did not
    release enough information for our services to identify you. You will need
    to contact appropriate staff at your organization to enable the release of
    the information, or alternatively use an Identity Provider with a more
    liberal policy.

    You may direct your technical staff to this infrastructure page for specific
    details on configuration and our policies regarding the information we receive.

    *NOTE:* You should also verify that your browser is not blocking cookies to
    the "shibboleth.net" servers as this will also trigger this message.

I did double-check my cookie settings, and they were allowing them.  So, unless 
there is some other way to login, I can't submit the issue at this time.
