Odd authentication issue with EzProxy

I think we may have figured this one out actually. It was poor communication on the part of the affected individuals. Looks like it is neither EzProxy nor Shib but some .htaccess and X-FORWARDED-FOR header confusion on the resource behind EzProxy.

If it didn't support long RelayState values, presumably it wouldn't
generate them, so I doubt that that's the problem.
