Well, it has apparently automagically fixed itself. ELON called me this morning to tell me that everything is working fine. They made no change on their side. I made none on mine. I hate things like this :(

Thanks, Chris. The URL is basically the same for this customer as all others with the exception of the EntityID embedded in the URL.

...provided that the users from this IdP are hitting the same URL as all
the working IdPs. If there's something different (http vs. https,
different vhost, different path, etc.) then the setting wouldn't be in
effect.
