All I can tell you is that the server name MUST be accurate (which does
NOT mean "matches entityID") and that the code will examine that
environment variable ahead of pulling it from SERVER_NAME.

The entityID is not the point, but if that's the hostname, then your
RequestMap does not have content settings in it that would cause the SP to
interject anything. You're not requiring a session basically.
