Your web server isn't setting the server name properly, so your settings
aren't mapping (note the bogus hostname in that URL in the log). I don't
recall the ins and outs of that for FastCGI, but the code implies that it
looks at SHIBSP_SERVER_NAME as an override, and then backs off to a
SERVER_NAME variable from the FCGI environment.
