We recently implemented an authentication plug-in to allow use of the 
rapid identity product for MFA. The development was sponsored by Bradley 
University who open sourced the result:

I can't say whether it would serve as a good example :), but it's 
probably one of the few third-party plug-ins around from what I understand.

I'm not sure what type of plug-in the OP was interested in developing, 
but I originally reviewed the bundled TOTP plug-in while working on this.
