Load Testing Highlighting CAS Bugs

FYI, I've found a couple of CAS protocol bugs during load testing in an
attempt to verify whether CAS protocol is affected by the bug in IDP-641.
Ironically, it appears that particular bug doesn't affect CAS due to
different error handling strategy. So far there's nothing that couldn't be
targeted at 3.1.1, and I'll try to get fixes committed in the next couple
days.
