Scott,
Having another go at building a debug build...
It seems there is yet another dependency missing: "boost". There is no mention of this on the NativeSPWindowsSourceBuild page.
All I actually need is a PDB file to allow me to debug using the debug-build DLL that actually ships with the SP installer. Is that available anywhere?

I don't have any strong ideas, I've gone through everything obvious, and you're using the default applicationId, so that rules out another set of weird possibilities.
Assuming the logs on both sides don't show anything unusual, I'm not coming up with anything obvious. If the Session handler shows them, they're extracted from the SAML, so that rules out anything to do with the IdP really.
