Authentication plugin for Shibboleth IDP 3

Not sure if this can help, but for a course I developed the following 
authentication plugin:
https://github.com/biancini/IdP3-TwoFactor

In the README you can find how to configure and enable it in the 
password authentication flow.
And the code is very small and so very easy to understand.
