When I discovered the joy that was <BeanManagedConnection> I pretty much
guessed that.  So really, the question is how quickly and actively do we
hurry <ApplicationManaged> out?

We obviously cannot kill it, but we can start with the documentation (done)
but we could probably hurry things along by putting a warning into the
parser....
