Logos& CSS in the distribution

As I was reviewing the various pages as part of IdP-101 I internalized what
I have known from some time: that the logos and CSS used by an IDP will be
served from the war file and so if people want to change them then they will
need to rebuild the war file.

Once I had got this this I realized that that was fine and that those things
are not likely to change frequently (unless the organization is blessed with
a hyperactive marketing department).

But I did wonder whether we might want to change the kitting and/or
installation slightly to put our default css and images into the edit-webapp
as well/instead as having it in war.

That makes it more obvious to the end user what needs to go where...
