The MDA doesn't really need to handle EntitiesDescriptor-level Extensions at all, in any kind of sophisticated way.  In most cases, there won't be any (as you can't really use KeyAuthority across federations) and so the first thing you do is blindly disassemble any aggregate into independent DomDocumentItem objects corresponding to the EntityDescriptors.
