Raw notes from DC meeting

Most of this is likely unintelligible to anybody but myself or Brent in
the current form, but the flows.txt notes may be somewhat more readable.
That's a breakdown of SAML 1/2 SSO as it pertains to the actions we think
we need to build, some demarcation points for where the Message handling
pipeline starts and stops, some boundary points for authentication,
session related code, error handling, etc.

We identified a number of areas of work left to do, but we did a pretty
good dive into all of the actions, all of the profile contexts needed, how
error handling might work, sequence of activity, etc.

I will be writing this up as best I can and eventually building classes to
reflect a lot of this.

We also reviewed by authentication context design and made a number of
changes to that which I will be updating the wiki to reflect.
