Thanks, John.  I have been able to reproduce the problem and have opened a JIRA case here:

	https://issues.shibboleth.net/jira/browse/MDA-87

The bad news is that I don't know what the problem is yet.  The good news is that I have narrowed it down enough to be able to think of a workround, which you can find in the JIRA case.  It would be a great help if you could try the workround in your situation and let me know if it works for you.
