Currently the OP plugin always issues id_token in the token response, whenever the *openid* scope is requested and granted for the RP. That’s in sync with the OIDC core spec [1] section 12 “Using Refresh Tokens”: it states that the token response might not contain id_token.

Anyway, we should provide a mechanism to control whether the id_tokens are issued via refresh_tokens or not.
