[JIRA] (OSJ-341) DiscoveryResponseBuilder defaults to wrong Namespace prefix and URI

The tests are tricky to fix because they seem to all route through a layer of indirection that relies on passing in the QName of the element to build so it never exercises the default build method. That's pretty pervasive to fix at this point and hopefully the general stability of this code will make that less of a problem.
