    Description: Create an action that adds the DoNotCache condition to all assertions in the outgoing response.  If the assertion does not already have a Conditions element, add it.  (was: Create an action that adds the consent URI to the outgoing response.

Need to determine where to get the consent URI from.  Probably use a fixed default and allow consent engine to indicate an alternative.)
